Then we prepare a detailed care plan. The plan may include a range of services—from determining eligibility for programs, coordinating and monitoring home health services, arranging for household maintenance and repairs, meal preparation, grocery delivery, transportation services—to finding the most appropriate retirement communities, assisted living, or nursing home care, if necessary.
